The Risks of Surgery

 

The indications, techniques, alternative treatments and anticipated results are discussed with each patient. My duty as your Doctor would not be complete until you are made aware of the possible reasonable risks, complications, and consequences of surgery.

Many activities of daily life – driving, flying, swimming, bicycling – carry a certain amount of risk of bodily harm. Both minor and major injuries can cause pain, deformities and suffering. Similarly, any surgical procedure carries some risk. Just as we cannot predict when or whether any of us will ever experience a stroke, heart attack, or other major medical condition in our lives, neither can we predict neither their occurrence before, during or after surgery. However, the occurrence of complications during and after cosmetic surgery is rare. Although many patients do not really want to know these possibilities, it is necessary for you to be informed and aware of them. Thus, in addition to the above, the following are some of the possible risks, complications and consequences associated with surgery in general and specific for phalloplasty, again very rare:

GENERAL

  • • Preoperative medication and local, regional, or general anesthesia.
    • Allergic reaction to one or more medication (s).
    • Unusual sensitivity reaction to one or more of the medication (s).
    • Death (if an anesthetist is in attendance, he or she will further discuss this).

SURGICAL PROCEDURES

  • • Bleeding
    • Hematoma (blood collection)
    • Infection, either localized or generalized, with or without abscess formation
    • Death
    • Shock
    • Amputation
    • Surface irregularities, or areas of elevation/depression
    • Collection of fluids such as hematoma (blood) or seroma, which contains reddish,
    yellowish to clear fluid; these may be long lasting and require repeated aspiration.
    • Persistent edema (swelling)
    • Thrombophlebitis – a blood clot can migrate and cause pulmonary emboli (i.e.,
    blood clots to the lungs).
    • Skin problems
    • Variations in pigmentation, with color either too dark or too pale in operated areas
    • Necrosis or sloughing, including skin breakdown, loss of tissue (skin and fat) due to poor blood supply.
    • Scars – either hypertrophic, keloid, dark or light
    • Nerve damage – sensory (numbness) or motor (paralysis)

SPECIFIC for MISL and FREE FAT TRANSFER PHALLOPLASTY

Because other surgeons use differing techniques and large studies indicating the rate of complications are not yet available, I can only speak for my procedure.

However since my experience is over 15 years and several thousands of phalloplasties, I feel these statistics are valid.

The following is a list of the more serious, although rare, complications that one should be aware of:

  1. 1. Complete, permanent erectile dysfunction that is uncorrectable – I am not aware of any of the above in my series, so a reasonable risk would be 1/2000. However, I have seen tragic results by other surgeons resulting in the above complication, again rare.
    2. Complete Permanent Uncorrectable Partial E.D. – Same as above.
    3. Correctable E.D. – 1/1000 is a reasonable figure.
    4. Permanent loss of full or partial sensation – 1/1000 – again a reasonable figure.
    5. Bleeding – during surgery – 1 /2000
    6. Bleeding – after surgery 1/700
    7. Infection – Deep 1/1500, Superficial 1/200
    8. Keloid or large visible scars or pigment problems – African American - less than 3%, Asian less than 3%, Caucasian undetermined
    9. Complete loss of fat transferred – 1/600 (usually able to be replaced).
    10. Incidence of lumps, or uneven appearance of fat – less than 5% - easily correctable.
    11. No gain in length – about 3-4% usually because instructions are not followed properly.
    12. Actual loss of length – less than 1%- again usually because instructions are not followed correctly or some unforeseen circumstances, e.g., bicycle or auto accident, etc.
  2. 13. Poor Outcome- If you as a patient and I both agree that you did not get a
    reasonably good result through no fault of your own- (less than 1/700) then this will be corrected free of charge.

The above figures are accurate to the best of my knowledge. Although this list is extensive, it is not exclusive. Other factors can play a role in the outcome and satisfaction you feel with the procedure, such as:

  • Emotional or personality changes and mental difficulties such as excitement or depression can develop even when the operation has been otherwise successful and can adversely affect the outcome.
  • Additional operations may be necessary to improve things.
  • SPECIFIC: - If you have any predisposing factors such as heart disease, high blood pressure, smoking, diabetes, etc., these will be discussed with you.
